Oh, I see that this form is specifically used for taxpayers who are getting ready to submit an offer and compromise with the IRS. This form is about eight pages long. However, in an effort to condense it and keep this video short for your viewing, I have gone ahead and prefilled out the form. Now, we're just going to go through what that looks like. First, let's start with Section 1, which is your basic personal and household information. In this section, you will put in your information, your spouse's information, your address, and whether or not you're married or unmarried. You should also indicate if you own your home, rent it, or have a different living situation. Additionally, you should provide your children's names, ages, and relationships. There are also two checkes here. The first one asks whether or not your children are claimed as dependents on your Form 1040. If they are not claimed as dependents, mark "no" here, and be aware that you will not receive credit for them during the offer process, unless there's court-ordered child support. The second check asks whether or not your children contribute to the household income. Moving on to the next section, which is employment information for wage earners. Here, you will provide your employment information and that of your spouse if applicable. Include how long you've been at your current job, whether or not you have any ownership interest in a business, and what your occupation is.
